{"id":8341,"date":"2022-05-24T08:49:34","date_gmt":"2022-05-24T05:49:34","guid":{"rendered":"https:\/\/www.bonalive.com\/en\/?p=8341"},"modified":"2023-05-04T08:32:45","modified_gmt":"2023-05-04T05:32:45","slug":"4-innovations-for-an-antibiotic-free-future","status":"publish","type":"post","link":"https:\/\/www.bonalive.com\/en-us\/news\/4-innovations-for-an-antibiotic-free-future\/","title":{"rendered":"4 innovations for an antibiotic-free future"},"content":{"rendered":"<h4><strong>Bone infection cases on the rise<\/strong><\/h4>\n<p>Bone infection, also called osteomyelitis, is an inflammatory disease of the bone marrow caused by infectious pathogens.<a href=\"https:\/\/apm.amegroups.com\/article\/view\/63390\/html#B1\" target=\"_blank\" rel=\"noopener\"><sup>1<\/sup><\/a> Bone infections are microbial infections that can be either acute complications following trauma or surgery, or chronic conditions developing slowly over time.<sup><a href=\"https:\/\/web.archive.org\/web\/20170211144856\/https:\/\/rarediseases.org\/rare-diseases\/osteomyelitis\/\" target=\"_blank\" rel=\"noopener\">2<\/a>, <a href=\"https:\/\/www.hindawi.com\/journals\/cripa\/2015\/129020\/\" target=\"_blank\" rel=\"noopener\">3<\/a><\/sup> Recurrent and persistent infections occur in approximately 40% of patients.<a href=\"https:\/\/journals.asm.org\/doi\/10.1128\/CMR.00084-17\" target=\"_blank\" rel=\"noopener\"><sup>4<\/sup><\/a> In children, the infection usually concerns the long bones of arms and legs, while in adults the spinal vertebrae, hips, and feet are most commonly affected.<a href=\"https:\/\/apm.amegroups.com\/article\/view\/63390\/html#B1\" target=\"_blank\" rel=\"noopener\"><sup>1<\/sup><\/a><\/p>\n<blockquote><p>Osteomyelitis remains a serious, potentially destructive infection<\/p><\/blockquote>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-8358\" src=\"https:\/\/www.bonalive.com\/wp-content\/uploads\/2022\/05\/Chronic-osteomyelitis_distal-femur.jpg\" alt=\"Osteomyelitis\" width=\"250\" height=\"250\" \/>Bone infection is a prevalent disease affecting males more often than females, especially after 60 years of age.<sup><a href=\"https:\/\/apm.amegroups.com\/article\/view\/63390\/html#B1\" target=\"_blank\" rel=\"noopener\">1<\/a>, <a href=\"https:\/\/web.archive.org\/web\/20170211144856\/https:\/\/rarediseases.org\/rare-diseases\/osteomyelitis\/\" target=\"_blank\" rel=\"noopener\">2<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4642868\/\" target=\"_blank\" rel=\"noopener\">5<\/a><\/sup> It was common before the invention of antibiotics and even though morbidity has dropped to 3%, it is still globally significant and the disease remains a challenge to treat.<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3763212\/\" target=\"_blank\" rel=\"noopener\"><sup>6<\/sup><\/a> In addition, in a study conducted in the United States, osteomyelitis cases were increasing and had doubled from the 1970&#8217;s until the early 2000&#8217;s due to factors including changes in diagnostics and a significant increase in diabetes-related and vertebral bone infections.<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4642868\/\" target=\"_blank\" rel=\"noopener\"><sup>5<\/sup><\/a><\/p>\n<p><sub><em>Picture: PET-CT of chronic osteomyelitis in the distal femur. Courtesy of Infection Unit, Maastricht UMC+, the Netherlands.<\/em><\/sub><\/p>\n<p>&nbsp;<\/p>\n<h4><strong>Infection sources include more resistant bacteria<\/strong><\/h4>\n<p>The major sources of bone infection are hematogenous spread, resulting from an infection that is transported through the bloodstream to a bone in a distant location, and direct pathogenic inoculation from trauma or surgery.<sup><a href=\"https:\/\/apm.amegroups.com\/article\/view\/63390\/html#B1\" target=\"_blank\" rel=\"noopener\">1<\/a>, <a href=\"https:\/\/web.archive.org\/web\/20170211144856\/https:\/\/rarediseases.org\/rare-diseases\/osteomyelitis\/\" target=\"_blank\" rel=\"noopener\">2<\/a><\/sup> If left untreated, the infection leads to progressive bone destruction and loss.<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4642868\/\" target=\"_blank\" rel=\"noopener\"><sup>5<\/sup><\/a><\/p>\n<p>Osteomyelitis is most commonly caused by the bacterium <em>Staphylococcus aureus<\/em>, but also <em>Staphylococcus epidermidis<\/em>, <em>Streptococcus pneumonia<\/em>, and <em>Streptococcus pyogenes<\/em> are common pathogens.<sup><a href=\"https:\/\/apm.amegroups.com\/article\/view\/63390\/html#B1\" target=\"_blank\" rel=\"noopener\">1<\/a>, <a href=\"https:\/\/web.archive.org\/web\/20170211144856\/https:\/\/rarediseases.org\/rare-diseases\/osteomyelitis\/\" target=\"_blank\" rel=\"noopener\">2<\/a>, <a href=\"https:\/\/journals.asm.org\/doi\/10.1128\/CMR.00084-17\" target=\"_blank\" rel=\"noopener\">4<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4642868\/\" target=\"_blank\" rel=\"noopener\">5<\/a> <\/sup>More resistant strains have emerged due to growing antimicrobial resistance (AMR), and the main pathogens are slowly changing from gram-positive to gram-negative bacteria.<a href=\"https:\/\/apm.amegroups.com\/article\/view\/63390\/html#B1\" target=\"_blank\" rel=\"noopener\"><sup>1<\/sup><\/a><\/p>\n<blockquote><p>Innovative non-antibiotic antimicrobial approaches are vital<\/p><\/blockquote>\n<p>Risk populations include people with diabetes, hemodialysis patients, and intravenous drug addicts.<a href=\"https:\/\/web.archive.org\/web\/20170211144856\/https:\/\/rarediseases.org\/rare-diseases\/osteomyelitis\/\" target=\"_blank\" rel=\"noopener\"><sup>2<\/sup><\/a> Bone infections of the foot are common in people with diabetes. The lifetime risk of people with diabetes to develop a foot ulcer is 34%, and more than half of these ulcers become infected<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28614678\/\">7<\/a><\/sup>. A further 20% of diabetic foot infections result in amputation<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/22619242\/\">8<\/a><\/sup>. \u00a0Specialists state that these amputations are largely preventable, yet 50% of these patients die within 5 years of the procedure.<sup><a href=\"https:\/\/www.medicinejournal.co.uk\/article\/S1357-3039(14)00296-5\/fulltext\" target=\"_blank\" rel=\"noopener\">9<\/a>, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16291066\/\" target=\"_blank\" rel=\"noopener\">10<\/a>, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26079161\/\" target=\"_blank\" rel=\"noopener\">11<\/a>, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32209136\/\" target=\"_blank\" rel=\"noopener\">1<\/a>2<\/sup><\/p>\n<h4><strong>Innovations for the treatment of bone infections<\/strong><\/h4>\n<p>Bone infections have traditionally been treated with large doses of pathogen-appropriate antibiotics, both intravenously and locally, and depending on the extent of the infection, with surgical cleaning of the affected area. Bone grafts are sometimes necessary as well.<a href=\"https:\/\/web.archive.org\/web\/20170211144856\/https:\/\/rarediseases.org\/rare-diseases\/osteomyelitis\/\" target=\"_blank\" rel=\"noopener\"><sup>2<\/sup><\/a> The diagnosis and treatment of osteomyelitis have improved over the previous decades with advances in bone regeneration, the development of new antibiotics, and improvements in vascular surgery and trauma care.<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4642868\/\" target=\"_blank\" rel=\"noopener\"><sup>5<\/sup><\/a><br \/>\n<img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-8393 alignleft\" src=\"https:\/\/www.bonalive.com\/wp-content\/uploads\/2022\/05\/Bacteria-adherence-1.png\" alt=\"\" width=\"300\" height=\"227\" \/><\/p>\n<p>Nevertheless, osteomyelitis remains a serious, potentially destructive infection, especially in diabetes-related diabetic foot infection cases. It is biodegradable, biocompatible, and nontoxic and displays antimicrobial activity. Chitosan is often combined with metal ions.<a href=\"https:\/\/journals.asm.org\/doi\/10.1128\/CMR.00084-17\" target=\"_blank\" rel=\"noopener\"><sup>4<\/sup><\/a><\/p>\n<ol start=\"4\">\n<li><strong>Peptides<\/strong><\/li>\n<\/ol>\n<p>Antimicrobial peptides (AMPs) are short proteins that form part of the human immune system. Some antimicrobial peptides, for example LL-37, demonstrate vast antimicrobial activity and promote bone regeneration.<a href=\"https:\/\/journals.asm.org\/doi\/10.1128\/CMR.00084-17\" target=\"_blank\" rel=\"noopener\"><sup>4<\/sup><\/a> Synthetic peptides called Synthetic Antimicrobial and Antibiofilm Peptides (SAAPs) kill antibiotic resistant strains within hours and are not associated with resistance development.
