Infection treatment is getting more difficult due to growing antimicrobial resistance (AMR). \u200b\u200bEvery year, 2 million people with diabetes get bone infections and 1.7 million patients undergo amputation.\u200b According to specialists, 80-85% of all amputations caused by diabetes are largely preventable <sup><a href=\"https:\/\/www.medicinejournal.co.uk\/article\/S1357-3039(14)00296-5\/fulltext\" target=\"_blank\" rel=\"noopener\">3<\/a>, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16291066\/\" target=\"_blank\" rel=\"noopener\">4<\/a>, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26079161\/\" target=\"_blank\" rel=\"noopener\">5<\/a><\/sup>.<\/p>\n<p><a href=\"https:\/\/www.diabetesfeetaustralia.org\/diabetic-foot-disease-is-a-top-10-cause-of-global-disability\/\" target=\"_blank\" rel=\"noopener\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-8053 size-full\" src=\"https:\/\/www.bonalive.com\/wp-content\/uploads\/2022\/01\/Diabetes-complications.jpg\" alt=\"\" width=\"671\" height=\"337\" srcset=\"https:\/\/www.bonalive.com\/wp-content\/uploads\/2022\/01\/Diabetes-complications.jpg 671w, https:\/\/www.bonalive.com\/wp-content\/uploads\/2022\/01\/Diabetes-complications-640x321.jpg 640w, https:\/\/www.bonalive.com\/wp-content\/uploads\/2022\/01\/Diabetes-complications-320x161.jpg 320w\" sizes=\"(max-width: 671px) 100vw, 671px\" \/><\/a><\/p>\n<p>1 out of 2 patients die within 5 years after amputation<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32209136\/\" target=\"_blank\" rel=\"noopener\"><sup>6<\/sup><\/a>. Novel antibacterial solutions can prevent these amputations. Prof. Piaggesi continued saying that especially patients with diabetes, who have a weak immune system, are particularly prone to infections, and the treatment of wounds in their lower extremities is more difficult. <em>\u00abWe have started using Bonalive&#8217;s products at our clinic and this new technology has enabled us to save many limbs, even among young patients\u201d, <\/em>he concluded<em>.\u00a0<\/em>(Helsingin Sanomat, 06\/2021)<\/p>\n<p>&nbsp;<\/p>\n<blockquote><p>\u00ab&#8230; this new technology has enabled us to save many limbs\u00bb Prof. Piaggesi<\/p><\/blockquote>\n<h4><\/h4>\n<h4>Treating diabetic foot osteomyelitis without local antibiotics<\/h4>\n<p>As the world is searching for new solutions to combat infections due to antimicrobial resistance (AMR), Bonalive set out more than a decade ago to develop a novel concept to treat and prevent bone infections without the need of local antibiotics. The Bonalive S53P4 bioactive glass was developed as a bone regeneration solution and due to the proven bacterial growth inhibiting properties, the use has expanded to bone regeneration applications in septic bone surgery.<\/p>\n<p>The first pilot study, with 11 patients, was published in 2010 and the CE mark for bone cavity filling in the treatment of chronic osteomyelitis was approved in 2011. Since then, 13 retrospective studies (376 patients), 3 prospective studies (37 patients) and 6 comparative studies with control groups (124 patients) have been published with our Bonalive products, as the first CE- marked solution in the world, to treat bone infections in orthopedics and traumatology without local antibiotics. The main benefit of using this mechanism, in contrast to the traditional means with antibiotics, is that no local antibiotics are needed, efficacy has been proven towards resistant bacteria (e.g., MRSE and MRSA) <sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26462989\/\" target=\"_blank\" rel=\"noopener\">7<\/a>, <a href=\"https:\/\/bmcinfectdis.biomedcentral.com\/articles\/10.1186\/s12879-018-3069-x\" target=\"_blank\" rel=\"noopener\">8<\/a>, 9<\/sup>, it has a broad-spectrum efficacy towards both gram positive and gram-negative bacteria <sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/17569007\/\" target=\"_blank\" rel=\"noopener\">10<\/a>, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/17619981\" target=\"_blank\" rel=\"noopener\">11<\/a><\/sup>, antibiofilm activity has been evidenced<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24957087\/\" target=\"_blank\" rel=\"noopener\"><sup>12<\/sup><\/a> and the mechanism does not promote resistance selection<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26228640\/\" target=\"_blank\" rel=\"noopener\"><sup>13<\/sup><\/a>.<\/p>\n<p>We have only started to understand what is happening at the interface of the technology (S53P4 bioactive glass) and biology (bone). As S53P4 is a bioactive biomaterial, a cascade of reactions is elicited that are thought to have an impact on multiple levels.<\/p>\n<p><strong>Properties of S53P4 bioactive glass:<\/strong><\/p>\n<p>&#8211; Bacterial growth inhibiting<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32552165\/\" target=\"_blank\" rel=\"noopener\">14<\/a>&#8211;<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32058256\/\" target=\"_blank\" rel=\"noopener\">15<\/a><br \/>\n&#8211; <\/sup>osteostimulative<sup><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1742706121001859\" target=\"_blank\" rel=\"noopener\">16<\/a>&#8211;<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/30194906\/\" target=\"_blank\" rel=\"noopener\">17<\/a><br \/>\n<\/sup><\/p>\n<p>Studies have also revealed the following properties:<\/p>\n<p><sup>&#8211; <\/sup>angiogenesis stimulation<sup><a href=\"https:\/\/helda.helsinki.fi\/handle\/10138\/237056\" target=\"_blank\" rel=\"noopener\">18<\/a>&#8211;<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24357515\/\" target=\"_blank\" rel=\"noopener\">19<\/a><br \/>\n&#8211; <\/sup>anti-inflammatory and immuno-stimulatory properties<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/11288090\/\" target=\"_blank\" rel=\"noopener\">20<\/a>&#8211;<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33389745\/\" target=\"_blank\" rel=\"noopener\">21<\/a><\/sup><\/p>\n<p>All these properties of the researched S53P4 bioactive glass are thought to contribute to the healing process of these challenging patients.<\/p>\n<p>&nbsp;<\/p>\n<h4><strong>How does bioactive glass help address these needs? Various types of antiseptics have been used to treat diabetic foot ulcers, but the available evidence does not support any beneficial effect for most of these<\/em><em>\u201d.<\/em><\/p>\n<blockquote><p>The first clinical experiences showed a significantly higher infection resolution rate (90% vs 61.9%) compared to Standard of Care.<\/p><\/blockquote>\n<p>We are very inspired by the first clinical experiences in Italy with S53P4 bioactive glass (<a href=\"https:\/\/www.bonalive.com\/en\/products\/bonalive-granules\/\" target=\"_blank\" rel=\"noopener\">Bonalive\u00ae granules<\/a>), as they showed a significantly higher infection resolution rate (90% vs 61.9%) compared to Standard of Care (SoC)<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S1067251620303409\" target=\"_blank\" rel=\"noopener\"><sup>22<\/sup><\/a>.
